Creating File Upload Forms

File Upload Forms can be used to collect documents from a candidate. Some examples of how this could be used is to collect a driver's license, resume, or a voided check. File Upload Forms are sometimes sent in conjunction with Paper Sign Docs to allow for the completed and signed form to be uploaded into the employee portal. File Upload Forms do not require a PDF.

FormClosed Any document that needs to be completed and signed by a Candidate creation and management allows self service flexibility for any admin users in BH Onboarding. Utilize the documentation in this section for instruction on best practices. BH Support is here to help with any questions about existing forms.   1. Go to Configuration > Forms
  2. Click the + next to File Uploads
  3. Enter the Form Name
  4. Add Form Instructions (if applicable)
  5. Click Add Form FieldClosed Any piece of information on a Form that the Candidate can fill out
  6. In Short Description, create a unique name for the field. 
    • Each field must have a unique name (identifier). The identifier must be unique across all forms, not just the current form.
    • No special characters can be used in the field name, except an underscore.
  7. Fill in the Question field. This could be a description of the file to upload
  8. Set the Field Type to File Upload
    • Only one file may be uploaded per form field question. If multiple files are needed, Conditional Logic can be utilized to determine which form field question should be displayed to the candidate.
  9. Select the File Type from the drop-down
  10. Click Create
  11. Select Create Form and Place Fields

The options available in the File Type drop-down determine the file name when it is uploaded. These can be customized by following these steps:

  1. Navigate to Configuration > Settings > System Settings
  2. Expand the Onboarding section
  3. In the setting Upload File Types add to the list (commas with no spaces separating options)
  4. Make sure the Enable File Uploads setting is ON
