Form Field Types

The following table contains a list of all the field types available when building forms.

Short Answer	Short text	Enter specialty area
Number	Numeric, no formatting.	Years of Experience
Money	Currency	Expected Pay Rate
Date	Prompts candidate to use calendar picker, formatted MM/DD/YYYY. Using the word “Date” in a field name will automatically format the date as YYYY/MM/DD.	Certification Expiration Date
Paragraph	Long text field to capture narrative response; allows box to scale as candidate types.	Explain why you are interested in this position
Dropdown	Pull down menu with several options to choose one response from. Appears similar to the dropdown menus on the form builder. Special characters cannot be used when initially creating responses for any of the multi-choice type fields; in order to include special characters first build the form, place the fields onto the PDF and save, then edit the form and add the special characters.	Which area most interests you? a. Machine Maintenance b. Machine Install c. Planning and Administration
Radio Group	Radio button options that allow users to select one answer. Special characters cannot be used when initially creating responses for any of the multi-choice type fields; in order to include special characters first build the form, place the fields onto the PDF and save, then edit the form and add the special characters.	In which area do you have the most medical experience? a. Neonatal b. Newborn/Infant c. Young Children d. Pre-teens/teens e. Adults
Yes/No	Checkboxes to select yes or no.	Do you have reliable transportation?
Single Checkbox	One checkbox field.	Check here to confirm/acknowledge the statement
Email	Captures email address; text entered must be in an email address format.	Alternate contact email
Phone Number	Telephone number, with dashes in formatting.	Alternate contact phone number
Checkbox Group	Multiple selectable fields - users may select more than one response.	Select all focus areas you have been involved in
Checkbox to Text	Presents a checkbox to the Candidate that will add text to the form when checked.	Are you exempt? When the box it checked for yes EXEMPT will write on the form.
Rating	Typically used for surveys; allows candidates to assign a rating value. Commonly used with requester fields in interview process.	Please rate the interviewee's technical skill level
Embedded Video	Uploaded video to form that candidate may play as they complete a form. This field only supports videos hosted on Vimeo.	Contact your Talent Platform representative to use this field type
File Upload	Allows user to upload a file Must be enabled by Talent Platform admin. There must be at least one File Upload Type in System Settings for this option to display.	Upload copy of voided check
Account Number	Used for capturing bank account and/or routing information; does not remove leading 0’s; Data entered in this field must be numeric. You can set a minimum and maximum length.	Account No. or Routing No. Recommended to use Mask setting with sensitive Account Number fields
Text - Unrestricted	Allows user to enter text as necessary, regardless of character types used.	Alternative to other Text fields such as a Short Answer field
Confirm Field	Allows a user to confirm information entered in a masked field; must relate a Confirm field back to the Unique Identifier and text must match exactly.	Confirm Account No.
Calculated	Will automatically place a value based on the formula put into this field using the results from either Money or Numeric fields on the same form.	numfield1+numfield2-(numfield3*numfield4)/numfield5
Typeahead Select	Functions similar to Checkbox Group (able to select more than one response from a set list) but adds in a space where the candidate can type in their answers and it will narrow down the options for them to select from. This can be a single or multi-select field.	State and provincial fields
